Staff text alerts: sign-up form

For GrowthWorks staff and peer supporters. Fill this out when you join. GrowthWorks house systems can text you short work alerts: a request to check on a common area, a request to approve a guest's door code, or a notice that a house system is offline. Alerts never include anyone's name or health information.

After you sign up, you will get one text confirming it: "GrowthWorks: you'll get house texts at this number. Msg frequency varies. Msg & data rates may apply. Reply HELP for help, STOP to opt out." You can reply STOP at any time. Signing up for texts is not a condition of your role; alerts can also reach you by phone call.

For the house manager: enter the number into the house system only if the box above is checked. Keep this form in the staff file.
